Created by Drashi Khendup with additional material by Eric Vogt, it was published in a paperback format in May 1997 [1][2].

The book is currently out-of-print and likely to remain so due to the ease of players creating digital aids themselves using current technology, and the wealth of player-created resources which have sprung up in the years since its release, particularly through initiatives such as the Miskatonic Repository.

The battle against the horrors of the mythos often leaves an investigator dead or insane. In either case, a paper trail has formed, one that can lead new, ignorant investigators into realms of unspeakable evil.

This supplement contains a large number of uneful [sic] form for use in
Call of Cthulhu® game. Death certificates, insanity certificates, patient records, and even fingerprint forms are all included and may be used immediately. Also included in a 25-page Sanitarium prescription pad ideal for live-action playing sessions. Use it to write "prescriptions" in a game, or just as something the players and keeper may scribble on.
