The stories mostly appeared for the first time in the collection, with the exceptions of Hugh B. Cave's "The Death Watch" (1939), Ramsey Campbell's "The Franklyn Paragraphs" (1973), Michael D. Winkle's "Typo" (1987), and Arthur William and Lloyd Breach's "The Return of the White Ship" (1989).

Cthulhu's Heirs won the Origins Award for Best Game-Related Fiction of 1994.
