Forever Azathoth and Other Horrors is an anthology of Lovecraftian fiction by American author and H. P. Lovecraft scholar Peter Cannon. Running to a total of 234 pages in a hardcover format, it was published by Tartarus Press in July 1999. Cover art was supplied by Jason Eckhardt, and it included an introduction by Steven J. Mariconda.
Not to be confused with the similarly-named anthology Forever Azathoth: Parodies and Pastiches (2011), published by Subterranean Press. Although the two share some of the same content, they are not identical.
