Stygian: Reign of the Old Ones is described as "a supernatural horror role-playing video game which takes place in the strange worlds of Howard Phillips Lovecraft". The game features a 2D graphical style with an art style "reminiscent of 20's illustrations". The setting takes place in the city of Arkham, displaced into an otherworldly dimension after the events of the "Black Day". The protagonist, either created by the player or pre-made, attempts to discover and follow clues leading to the mysterious "Dismal Man", In this endeavor they can recruit and be joined by other characters, aligned by common goals, circumstance, or as a paid henchman.

This game features survival and crafting mechanics, in which the characters must manage health, hunger and sanity. Health and hunger are managed by food and medicine but sanity is managed by the choices made. Each character has a belief system, when that character makes decisions that align with that belief system they recover sanity. Sanity loss along with a host of other causes can inflict mental illness upon a character which can change dialogue options, alter the characters perception of reality and more.

Combat is turn-based and tactical: making use of a hex-grid, characters can use melee weapons, guns, and spells in combat with the various forces in Arkham including members of a Cthulhu-worshiping cult, the Mob, Mi-Go and other creatures. Combat results in angst accumulation, at particular thresholds of angst the character gains a permanent de-buff, incentivizing careful decision making and avoiding unnecessary combat. The majority of combat is optional and much of it can be escaped rather than fought to the death.
